Community Skilled Health Care Centre – Warren, OH

Are you a licensed Occupational Therapist available to step in and make an immediate impact? We are urgently seeking a PRN OT to help with evaluations, progress notes, and recertifications at Community Skilled Health Care Centre in Warren, OH.

Your Role :

Conduct timely and accurate resident evaluations.
Complete progress notes and ensure proper documentation.
Handle recertifications to maintain continuity of care.
Collaborate with our interdisciplinary team to support residents' goals.

Requirements :

Current Ohio OT license in good standing.

Experience in skilled nursing or long-term care preferred.

Ability to step in quickly and efficiently manage required tasks.
