We are looking for a Cognitive Rehabilitation Therapist to work with adults with brain injuries or other neurological challenges during weekday hours. Possibility of FT employment in the future.
The cognitive rehabilitation therapist works with the client on the attainment/re-attainment of cognitive skills in the client's home and community utilizing and strengthening attention and concentration; processing and understanding information; memory; communication; planning and organizing; reasoning, problem-solving, decision-making and judgment; controlling impulses - to achieve functional changes by:
- Reinforcing, strengthening, or establishing previously learned patterns of behavior
- Establishing new patterns of cognitive activity or mechanisms to compensate for impairments
- Interventions that help the client be as independent as possible in their home and community
The therapist will always ensure the client’s safety while in the community and help facilitate appropriate community involvement and interaction.
Reports to: Director of Community Skills Program®
JOB RESPONSIBILITIES:
- Participate in standardized assessments/evaluations to determine individual strengths, weaknesses, learning strategies, and potential.
- Based on these assessments, develop, and implement individualized treatment programs to address identified needs.
- Conduct individual cognitive rehabilitation sessions in the clients’ homes and community per Individual Service Plan and Individualized Program Description (IPD).
- Develop specific, measurable home-and community-based goals to improve cognitive skills that promote client independence.
- Communicate with other members of the individual’s treatment team.
- Complete all required documentation such as assessments, daily progress notes, monthly reports, quarterly reports, and checklists per established due dates.
JOB QUALIFICATIONS:
- Bachelor’s degree in psychology, social work, education, or other human services field, or other area of concentration with required experience
- CBIS preferred or CBIS eligible
- Minimum of 1 year experience working with adults with TBI or intellectual disabilities
- Proficient knowledge and experience with information technology
- Strong analytic, organizational and time management skills
- Excellent written and verbal skills
- Must have reliable transportation
LICENSES AND CERTIFICATIONS:
- Valid Driver’s License
