POSITION TITLE: Program Coordinator – Behavioral Health
JOB TYPE: Regular, Full-Time, Non-Exempt, Union, Benefitted

SUMMARY:
Under general supervision of the Program Manager, the Program Coordinator is responsible for coordination and implementation of the assigned Agency’s programs. Responsibilities include quality client services, well-coordinated community education efforts and a contribution to meeting contract requirements. The Program Coordinator collaborates with public and community-based agencies, as needed. Services are provided throughout Santa Clara County.

Education and Experience:
Required:
  • Bachelor’s degree or equivalent and four (4) years direct client work experience in a related field.
  • Experience working with mental health and/or substance abuse issues.
Preferred:
  • The individual will preferably be bilingual and bicultural.
  • Certified Alcohol Drug Counselor (CADC).
Knowledge:
Required:
  • Empowerment, advocacy and support techniques for victims/survivors of violence, people with mental health disorders or substance abuse issues.
  • Working knowledge of wellness and recovery concepts, Medi-Cal regulations and program documentation.
  • Working knowledge of case management, group and individual counseling, and linkage-referral.
Preferred:
  • Working knowledge of community resources in Santa Clara County and surrounding areas required.
  • In collaboration with various County and other service providers, experience working with systems to meet requirements of the Agency and contracts.
Special Requirements: Must be able to receive a criminal records clearance as required by specific programs, licensing regulations and Community Solutions practices. Position requires an automobile to perform routine job duties.
License: Possess a valid California driver’s license, current car insurance and a good driving record as documented by a DMV report and consistent with the Agency’s liability insurance requirements.
What You'll Be Doing in the role of Program Coordinator – Behavioral Health: The following are examples of essential job duties for this position. The omission of specific statements of the duties does not exclude them from the classification if the work is similar, related, or a logical assignment for this classification. Other duties may be required and assigned to support the program and Agency.
  • Responsible for coordinating daily activities and implementation of programs to ensure the needs of clients are met and services meet contracts.
  • Provides support and guidance to staff, interns, and volunteers. Assigns, schedules and monitors caseloads in conjunction with the Program Manager.
  • Coordinates all new referrals, assigns rehabilitation counselor and coordinates client care with the County system. Handles phone calls, faxes, tracks referral on spreadsheet. Responsible for making sure referral procedures are followed and all necessary information and contact with clients runs smoothly.
  • Coordinates intakes and discharges of clients for program. Opens and closes charts for program. Coordinates psychiatric services with treatment team.
  • Coordinates case conferences and family meetings for team.
  • Compiles program data outcomes and reports for Managers in a timely manner.
  • Reviews charts on a monthly basis with Program Manager/Rehabilitation Counselor to ensure audit ready charts.
  • Ensures programs implement and maintain current policies/procedures and recommends improvements for intern team as needed.
  • Responds to phone calls and emails within 24 hours on business days unless extenuating circumstance exist. Resolves outstanding issues.
  • Conducts screening, intake and assessments as required. Assesses clients’ needs and goals to determine appropriate resources, including the referral to professional services, based on client and program needs. Promptly implements individual and family Transformational Care Plans, maintains detailed documentation and monitor's progress.
  • Provides direct services, crisis intervention, advocacy, case management and linkage/referral services using Evidenced Based practices when needed.
  • Completes detailed program documents in accordance with agency, program and regulatory requirements.
  • Maintains all charts within Agency and/or Medi-Cal regulations.
  • Participates in team meetings in a strength-based manner – building trust and addressing concerns.
  • Attends and participates in Agency meetings and contributes to continuous quality improvement activities to enhance team and program development.
  • Consistently uses strength-based and solution focused problem-solving skills.
  • Develops and maintains collaborative relationships with appropriate community organizations and system providers based on client and program needs. Create materials to foster community awareness about program services and goals.
  • Coordinate program community events, provide program education, training and presentations to staff, interns, volunteers, clients, families and the community.
  • Teaches life skills, assists individuals and families with educational and support activities, accesses appropriate community services and rehab counseling as needed.
  • Maintains excellent customer service skills with internal and external customers.
  • Successfully completes all training requirements for this position to ensure agency and contract compliance.
  • Employee must have the ability to operate a motor vehicle to provide daily transportation of clients and/or other assigned Agency business.
  • Frequent travel required.
  • Participates in on-call and after-hours rotation as assigned. Flexible schedule may be required to work some evenings and weekends.
  • Performs first aid as needed based on training and certification.

Physical Requirements:

Sit for extended periods; frequently stand and walk; normal manual dexterity and eye-hand coordination; lift and move objects weighing up to 25 lbs.; corrected hearing and vision to normal range; verbal communication; use of office equipment, including computer, telephone, calculator, copiers, and FAX.
Work Environment and Location: This position is located within commutable distance to our Gilroy headquarters or San Jose office and is open to a hybrid work schedule, with occasional needs to be able to travel into our offices and work in the evenings or weekends.
