Community Support Network is an expanding non-profit organization committed to combating homelessness in Sonoma County. Our mission focuses on fostering a compassionate, healthy, and safe environment that empowers individuals at risk of homelessness to achieve stability and realize their full potential. Our shelter program offers temporary accommodation for men and women experiencing homelessness or those at risk of becoming homeless, specifically targeting individuals who are actively engaged in services provided by Sonoma County Behavioral Health. The shelter is housed in a spacious historic home in Santa Rosa, complete with a yard, providing 13 beds for clients of Sonoma County Behavioral Health Services. This facility serves as a vital steppingstone toward transitional and permanent housing solutions.
Essential Functions:
The responsibilities associated with this program include, but are not limited to:
- Resident Relations: Maintaining positive, supportive, and respectful interactions with residents while assisting them with meeting their treatment goals.
- Supportive Services: Providing educational and supportive services within a social rehabilitation framework.
- Daily Living Activities: Assisting residents in completing daily living tasks.
- Group Activities: Organizing and facilitating group activities to foster community engagement among residents.
- Medication Management: Overseeing medication administration and ensuring compliance with prescribed regimens.
- Documentation: Keeping accurate records, including progress notes for clients, incident reports, and any special reports as necessary.
- Guidelines Compliance: Adhering to established guidelines, policies, and procedures while supporting residents.
- Crisis Intervention: Implementing crisis prevention strategies and interventions using a trauma-informed approach.
- Chart Reviews/Filing: Conducting regular reviews of client charts and maintaining organized filing systems.
Minimum Education/Requirements:
- Must be 18 years of age or older.
- A high school diploma or GED is required; an associate degree is preferred, although relevant experience in the social services field and life experiences are valuable assets.
- Strong observation and listening skills are essential.
- Ability to adapt and excel in a dynamic work environment.
- Capable of establishing and maintaining professional boundaries while interacting with residents; must possess strong interpersonal skills and demonstrate compassion while adhering to guidelines and policies regarding resident interactions.
