Community Systems, Inc. (CSI) is a non-profit organization offering residential and related supports to persons with developmental disabilities. CSI has an immediate need for a Full Time Health Care Coordinator position to provide direct clinical care and other supports to persons in their own homes.
Position: LPN HEALTHCARE COORDINATOR (Full Time)
The role of the Healthcare Coordinator is to assist in achieving CSI’s mission by ensuring that the individuals supported by CSI receive high quality health services. This includes ensuring early evaluation of health/dental needs, timely implementation of required treatment and the review of CSI policies and/or procedures to assure that the services rendered meet or exceed all applicable standards of care.
Duties of the LPN Healthcare Coordinator shall include, but are not limited to:
- Provide overall healthcare coordination to persons supported in their own homes. This includes establishing health profiles, identifying health risks and ensuring that persons supported have the opportunity to choose healthy lifestyles.
- Provide leadership, guidance and training for staff to effectively maintain the health status of persons supported and to ensure accurate and timely implementation of medical orders.
- Oversee the creation and maintenance of persons’ medical records to ensure compliance with state and internal regulations and policy.
- Participate as a member of interdisciplinary team to advocate for the persons supported.
Requirements:
- Active licensure in the state of Massachusetts as an LPN
- At least two years recent nursing experience in an acute or long-term care setting
- Experience working with individuals who have intellectual disabilities preferred
- Ability to work independently and manage a flexible work schedule
- Excellent written and oral communication skills, computer/word processing skills
- Valid U.S. vehicle driver’s license.
