Education/Experience:
- Current NJ Home Health Aide certification
- Valid Driver’s License required
- One year exp. in home health preferred
Job Summary: To provide care by performing activities related to personal care and maintenance, daily living and ensures a clean, safe home environment.
Job Description:
- Provide patients with help moving in and out of beds, baths, wheelchairs, or automobiles
- Accompany patient to doctor’s office or hospital for treatment.
- May assist with dressing, grooming, bathing and other personal care or activities of daily living.
- Assists patient with prescribed exercises
- Prepares nutritional and/or personalized diet requirements
- Maintain records of patient care, condition, progress, or problems to report and discuss observations with supervisor or case manager.
