JOB DESCRIPTION
JOB TITLE: Case Manager for Adult Care Facility (ACF)/Assisted Living Program (ALP)
MAIN FUNCTION: Liaison with community and other health facilities
REPORTS TO: PRC Administrator
DUTIES AND RESPONSIBILITIES:
- Will review admission paperwork with residents and/or resident representative upon admission to the ACF and ALP level of care.
- Will perform initial and periodic evaluations, at least once every twelve months for the ACF and every 6 months for the ALP, to review theneeds of residents and the capability of the facility program to meet those needs.
- Will orient new residents and their family members to the daily routine and will assist residents to adjust to life in the facility.
- Will assist residents to maintain family and community ties and encourage resident participation in facility and community activities.
- Will arrange for services from public and private sources for income, health, mental health and social services and will assist residents in making application for and maintaining income entitlements and public benefits (SSI, Medicaid).
- Will assist the resident in the completion of the Medicaid application and/or SSI interview.
- Will assist residents in obtaining and maintaining a primary physician or medical care for management of individual needs.
- Will assist the resident in scheduling medical appointments and arrange transportation if necessary.
- Will coordinate and follow through on all necessary paperwork to assure compliance with all the rules and regulations as mandated by DOH.
- Will assist residents in making arrangements to obtain services, examinations and reports needed to maintain or document the maintenance of the resident’s health or mental health, including health and mental health services, dental services and medications.
- Will provide information and referral according to resident needs. Will assist residents in need of alternative living arrangements to make and execute sound discharge or transfer plans.
- Will assist residents to participate in facility operations with recommendations or grievances. Will be liaison for resident council.
- Will conduct tours of the facility, in the absence of the Director of Business Development.
- Will assist residents with internal or external services as necessary.
EDUCATION: A Bachelor’s or Master’s degree from an accredited college or university in a field related to social work and one (1) full year of experience in the provision of services to a dependent adult population; or an AAS from an accredited college or university in a field related to social work and three (3) years of experience in the provision of services to a dependent adult population;
EXPERIENCE: Educational degree AAS/BS/MSW as stated above.
CONTACTS: Daily contact with residents and the professional and non-professional members of the community.
WORKING CONDITION: Well ventilated, well lighted surroundings, congenial atmosphere.
PHYSICAL DEMANDS: Considerable walking, standing, and sitting.
