Special Education Transitions Facilitator- 
Onslow

Full-Time; 10 Month Contract

The Special Education Transitions Facilitator is responsible for supporting the instruction of direct services to students (ages 14-22) with disabilities. This includes understanding and being able to communicate individually or in groups and to facilitate learning of topics related to transitioning to adulthood. 

Responsibilities

• Supporting a caseload of 3-4 high schools across wake county.
• Creating lesson materials, data entry, billing and classroom support.

• Associates Degree in Psychology, Special Education, Rehabilitation or a related human services field preferred. 
• Experience working with students with developmental disabilities. Equivalent experience will be considered in lieu of education.   

Minimum Qualifications

• Must have valid NC Driver’s License and driving record that adheres to Agency standards.
• Must be able to work in various schools across the county.
• Must pass background check and drug screening.

Physical Requirements

• Must be able to lift 10 - 25 lbs.
• Must be able to handle sitting and standing for extended periods of time

• Pay Range: $17.00 -18.50 / hour
