Overview

The CommUnityCare Centering Pregnancy and Centering Parenting programs offer an alternative care model focused on group visits and education, which promotes positive health outcomes and patient satisfaction for pregnancy and pediatric care. These programs are nationally accredited by the Centering Healthcare Institute. The Medical Assistant - Centering Facilitator serves as co-facilitator of CommUnityCare’s Centering Pregnancy and Parenting programs. The Medical Assistant Centering Facilitator works in close collaboration with providers, clinical staff, and the Centering Steering Committee in order to ensure adherence to program operations and standards.

Responsibilities

Essential Duties (at least 5 that are non-negotiable duties and are absolutely pertinent to successfully completing the job without accommodations):

Primary Accountabilities:

• Co-Facilitates with licensed provider Centering Pregnancy and/or Parenting groups and discussions. • Preparation of centering groups including chart preparation and organizing materials and supplies for each group and session. Responsible for managing supplies and inventory. • Complete all direct and indirect care documentation in timely manner and ensure patient records are current and complete.• Oversight of recruitment, participation, retention and direct point of contact for patients in Centering Pregnancy and Centering Parenting groups. • Responsible for reporting information, assist with monitoring and maintaining the program's database to reflect participant delivery outcomes, benchmark data, and other indices as specified through awarded grants.• Collaborate with key players to ensure coordination and oversight of programming schedules. • Communicate and work in conjunction with other CommUnityCare centering sites. Back up facilitator for other sites as needed.• Collaborate with direct supervisor and co-facilitator (provider lead) to assess, create and introduce new program curriculum. • Assist direct supervisor with community outreach as needed, including but not limited to managing social media platforms, email correspondence and community events.• Maintain all licensure and/or certificate requirements.• Support direct supervisor with the implementation of program to new sites and the site approval processes. • Perform other duties as assigned. • Attend all practice leader and /or clinic site meetings regularly. These may be related to quality assurance, patient care and other related topics. • Ensure all tasks provided and associated with patient care, patient administrative processes and related duties comply with all regulatory and accreditation standards including The Joint Commission and CommUnityCare Standard Operating Procedures and CommUnityCare Policies and Procedures.

Knowledge/Skills/Abilities: 

• Excellent verbal and written communication skills necessary.• Strong analytical and interpersonal skills necessary.• Ability to prioritize, as well as effectively re-prioritize as required by changing schedules and deadlines.• Ability to organize and pay close attention to detail.• Group facilitation experience preferred.• Ability to work independently and unsupervised.• Knowledge of the Centering model preferred.• Spanish speaking preferred, not required.

Qualifications

MINIMUM EDUCATION: 

• Graduation from high school or equivalent. • Medical Assistant Diploma and/or National Medical Assistant Certification verifying graduation from a Medical Assistant Program or equivalent military healthcare experience.

 

MINIMUM EXPERIENCE:

Completion of a medical assistant externship.

 

REQUIRED CERTIFICATIONS/LICENSURE: 

Current Health Care Provider Cardiopulmonary Resuscitation (CPR) certification through American Heart Association or American Red Cross. Will be required to maintain a current CPR certification during employment.
