What are the responsibilities and job description for the LPN/CMA position at CommWell?
GENERAL REQUIREMENTS
• Work-up of patients (vital signs, weight, screenings, etc.)
• Performs fingerstick hgb & lead.
• Performs urine analysis and UCG’s.
• Performs visual screening using Snellen’s eye chart.
• Assists provider with pelvic and breast examinations.
• Notifies patients of lab results.
• Sends missed appointments and follow-up letters to patients
• Performs waived testing.
• Performs electrocardiograms.
• Performs phlebotomy.
• Assists with input of patient information into lab computer.
• Administers medication and vaccines per physician’s order.
• Assists with packaging of lab specimens for outside reference labs.
• Attends continuing education pertinent to job related issues as appropriate.
• Responsible for promoting the collaboration for the patient’s continuum of care with all members of the PCHH Care Team to:
o Improves or maintains the patient’s social, behavioral, functional, and physical health status.
o Assists with closing clinical gaps in care and ensure consistent continuity of care.
• Participates in case reviews during PCHH huddles:
o Daily
o Multidisciplinary
• Other duties as assigned (X-rays if trained).
Required Skills
CERTIFICATIONS AND LICENSURES:
• BLS Certificate from American Red Cross or American Heart Association
• Current Medical Assistant license in the State of North Carolina
• Clean driving record with current NC driver’s license and approved by corporate vehicle insurance vendor for driving privileges
