What are the responsibilities and job description for the Wellness RN position at CommWell?
GENERAL REQUIREMENTS
• Performs Annual Wellness Visits for eligible patients following CMS guidelines and ensure timely documentation and collaboration with supervising provider/PCP.
• Develop individualized preventive plans of care and advanced care plan in collaboration with supervising provider/PCP
• Provide Case Management services for the ACO top 10 patients and other high risk managed care patients as indicated
• Adheres to the CommWell Health and departmental attendance and punctuality guidelines.
• Performs all job responsibilities in alignment with the core values, mission, and vision of the organization.
• Performs other duties as required and completes all job functions as per departmental policies and procedures.
• Maintains current knowledge in present areas of responsibility (i.e., self-education, attends ongoing educational programs)
• Attends staff meetings and completes mandatory education, maintains required licensure status.
• Demonstrates competency at all levels in providing care to all patients based on age, sex, weight, and demonstrated needs. For non-clinical areas, has attended training and demonstrates usage of age-specific customer service skills.
• Wear protective clothing and equipment as appropriate.
• Triage’s patients to determine order of need, consulting with providers as needed to promote continued patient flow through the medical system. For patients triaged and not seen the same day, supervises the collection and recording of vital signs and other pertinent patient data.
• Completes nursing admission of each patient according to protocol.
• Carries out patient’s plan of care within health center policy, procedures, and legal limitations. Performs nursing procedures for minor treatment (such as cleansing of wounds, bandaging, suture removal, giving injections) at the direction of the medical provider or supervisor according to standard protocols.
• Delegate’s lab assignments to nursing in assistance to overall lab requirements per clinic regulation.
• Performs waived testing as needed.
• Develops and updates existing nursing protocol to ensure a quality standard of care.
• Maintains a medical supply inventory of needed equipment instruments and ensures procurement of adequate supplies.
• Maintains and implements a safety program that monitors the following:
o Equipment maintenance and annual calibrations
o Universal precautions
o Disposal of hazardous waste
• Provides psychosocial interventions with individuals, families, and groups.
• Coordinate’s client care with relevant agencies to facilitate implementation of treatment plan.
• Implements preventative treatment and follow-up programs to meet the needs for the client population.
• Collaborates with all other staff within the health center to provide and maintain primary care of clients.
• Participates in staff conferences to devise patient care plans, to revise and update clinic service and CQI program.
• Carries out proper procedures for basic life support, fire, disaster, and infection control.
• Participates in case reviews during AMH/PCHH/Caring Eagle Huddles:
o Daily
o Multidisciplinary
• Other duties as assigned
Required Skills
CERTIFICATIONS AND LICENSURES
• BLS Certificate from American Red Cross or American Heart Association
• Registered Nurse (RN) with current NC license
• Clean driving record with current NC driver’s license and approved by corporate vehicle insurance vendor for driving privileges
