Overview
The Companies of J.J. Young is seeking a Program Coordinator for our non-profit client in Albany. The client supports underprivileged communities in New York by providing outreach, support, counseling, and community resources. Candidates must have independent means of travel to reach events. Candidates should also be familiar with the needs and issues face by Transgender and Gender Non-Conforming (TGNC) communities.
Position Overview
The Program Coordinator will lead the Transgender Care program through transformative initiatives, providing life-changing services, and building inclusive health access. The program focus is on HIV/STI/HCV prevention, risk reduction, and education. The primary scope of this role includes overseeing intervention programs, supervising staff, and collaboration with community partners to improve healthcare access for TGNC individuals. This position offers a combination of program development, advocacy, and direct service work.
Responsibilities
Program Leadership
- Develop, implement and oversee HIV prevention and health services
- Lead and facilitate intervention programs
- Expand HIV/STI/HCV testing services and increase the number of TGNC individuals who are aware of their status and connected to care
- Plan and execute social marketing campaigns
Outreach and Partnerships
- Build and strengthen partnerships with local agencies, community-based organizations, and coalitions advocating for TGNC health equity across NYS
- Lead digital and in-person outreach efforts to increase program visibility and accessibility
Case Management & Health Services
- Conduct risk assessments, HIV/HCV testing, and counseling
- Provide patient navigation and direct case management, ensuring clients receive comprehensive support, referrals, and linkage to care
- Supervise HIV/HCV Test Counselor, ensuring quality assurance for testing, documentation and data reporting
Staff Supervision and Compliance
- Provide leadership and supervision to a Case Manager, community Engagement Specialist, and Peer Advocate
- Ensure adherence to HIPAA and HIV confidentiality laws
- Maintain quality assurance across all program services
Qualifications
- Bachelor' degree required (Public Health, Social Work, Human Services, or another related field)
- 2 years of experience in HIV prevention, counseling, education, or social services with a focus on LGBTQ communities
- Strong knowledge of HIV/STI/HCV services, PEP/PrEP, gender-affirming care, and sexual health
- Experience with case management, risk assessments, and psychosocial support
- Excellent communication and facilitation skill, with comfort leading group interventions
- Ability to work flexible hours, including evenings and weekends as needed
- Valid drivers license and independent transportation for outreach and program needs
- Fluency in Spanish (Preferred)
- HIV Counseling and Testing Certification or willingness to obtain
- Experience managing community outreach programs and event coordination is preferred
