What are the responsibilities and job description for the Virtual Mental Health Resource and Discharge Planner position at Compass Health Center?
The Resource and Discharge Planner (RDP) is passionate about working with Young Adults and Adults to deliver compassionate care and provide them with a strong resource network. The RDP will be responsible for meeting with patients to discuss structure and time management. The planner will be responsible for facilitating aftercare planning with patients as well as supporting primary therapists with administrative tasks as needed. In addition, this role will be responsible for reaching out to local resources to establish referral opportunities for Compass patients.
What You'll Do:
- Actively pursue various community programs and events to pass along to patients
- Develop relationships with local resources, schools, volunteer sites, and therapists
- Manage referral lists and ensure accurate information regarding available appointments
- Manage weekly and monthly outreach goals as indicated by manager
- Work closely with Director/ Associate Director of Partnerships to implement outreach strategies
- Maintain adherence to CRM-based tracking of all relationships
- Teaching executive functioning strategies & coping skills
- Providing vocational support & transition planning for Young Adults/ Adults
- Empowering patients to create daily and weekly goals aimed at increasing activity levels, structure, and meaning in their everyday lives.
- Communicate and collaborate with the treatment team to identify the needs, goals, and progress of each patient
- Round with the Over 18 teams as clinically indicated to discuss patients
- Meet with patients regularly to follow up on resource goals (this will vary from patient to patient depending on what the patient is working on and level of functioning)
- Meet each patient where they are at and work together to develop an individualized resource plan. Plans may include (but are not limited to): school, jobs, volunteer work, positive recreational activities, ongoing supports (groups, AA/NA/OA, outpatient providers), social activities, exercise plans etc.
- Work with patients to create daily and weekly activity plans/schedules
- Create detailed follow up plans with each patient, including their resource related homework and daily tasks for the week. Consistently follow up with patients to help them identify progress and barriers.
- Address barriers and problem solve with patients
- Use motivational interviewing to strengthen the patient’s motivation for and movement toward goals by exploring their own reasons for change.
- Work closely with the treatment team to prepare patient for a successful discharge (ensuring that there are aftercare supports set in place and assisting patient with planning for the transition)
- Conduct clinical intakes and / or groups as needed
- Other duties, as assigned
Who You Are:
- Master’s degree in counseling, psychology, social work, or related field
- Excellent communication skills and time management skills
- Ability to problem solve
- Organizational skills, flexibility, knowledge of coping skills/core skills taught in groups
- Knowledge of motivational interviewing
- Ability to think on the spot and outside the box to find creative solutions
- Ability to manage numerous patients with varying needs
- Ideally possess a core skill set of Cognitive Behavior Therapy (CBT) and Dialectical Behavior Therapy (DBT)
