Job Overview

We are seeking a dedicated Nurse (RN) to join our Assertive Community Treatment (ACT) Team. Key Responsibilities

• Conduct psychiatric assessments and evaluate physical health needs of clients.
• Collaborate with clients and interdisciplinary teams to develop, implement, and adjust comprehensive treatment plans.
• Administer and manage medication in collaboration with the team psychiatrist, ensuring integration into individualized treatment plans.
• Provide education and support to clients and their families regarding mental health conditions and medication management.
• Participate in thorough assessments, including psychiatric history, mental status, and overall health evaluations.
• Attend daily meetings to participate in treatment planning and review processes.
• Maintain accurate documentation in accordance with agency policies, including progress notes and treatment plans.
• Offer on-call crisis intervention during evenings, nights, and weekends as necessary.
• Assist clients in navigating medical appointments and understanding their health needs.
