Key Responsibilities
- Manage a caseload of up to 250 Healthcare Home patients.
- Conduct health screenings with assigned clients to assess their medical needs.
- Collaborate with clients to develop individualized treatment plans and healthcare goals, particularly for those with co-occurring chronic conditions.
- Provide ongoing education and support to clients regarding their medical conditions, treatments, and medications.
- Consult with community and office-based staff on identifiable health conditions to enhance care delivery.
- Maintain communication with medical providers to share significant health information regarding clients.
- Monitor Health Information Technology (HIT) tools and reports to track treatment alerts and hospital admissions/discharges.
- Analyze monthly care manager reports, focusing on key population indicators and collaborating with healthcare providers as needed.
- Coordinate Metabolic Syndrome screenings and follow up with clients who have positive screens.
- Perform Medication Reconciliations for clients recently discharged from hospitals.
- Collaborate with other social and human service agencies to ensure holistic care for clients.
- Participate as a member of a multi-disciplinary team, advocating for culturally competent services tailored to the diverse needs of clients.
- Travel to neighboring clinics for nursing coverage and training as required.
- Proficient in Microsoft Office Suite and Electronic Medical Records.
- Strong communication and interpersonal skills, enabling effective teamwork and client interaction.
- Ability to operate within a multi-disciplinary team environment.
- High School diploma or GED required.
- Practical Nursing or Associate of Nursing degree is required.
- Bachelor's degree in nursing is preferred.
- A Licensed Practical Nurse (LPN) license is required; Registered Nurse (RN) license is preferred.
