Key Responsibilities
- Manage a caseload of up to 250 Healthcare Home patients.
- Conduct health screenings and assist in the development of individualized treatment plans for clients with co-occurring chronic diseases.
- Monitor clients’ medical requirements and provide necessary education on health conditions and treatment options.
- Consult and collaborate with medical providers regarding significant health information.
- Perform medication reconciliations for clients recently discharged from the hospital.
- Engage with other human/social service agencies to ensure holistic services for clients.
- Participate as a vital member of a multi-disciplinary team, advocating for culturally competent services tailored to the unique needs of each client.
- Travel to neighboring clinics for nursing coverage and training as necessary.
- Proficiency in Microsoft Office Suite and Electronic Medical Records.
- Strong communication and interpersonal skills.
- Ability to work collaboratively within a multi-disciplinary team framework.
- Commitment to delivering culturally competent healthcare services.
- High School diploma or GED is required.
- Practical Nursing or Associate of Nursing degree is required; a Bachelor’s degree in Nursing is preferred.
- Licensed Practical Nurse (LPN) certification is required; Registered Nurse (RN) certification is preferred.
