Compass Health Network is actively seeking a dedicated Hospital Access Liaison (HAL) to support individuals who have recently been discharged from the Emergency Department (ED) or inpatient units. This role plays a critical function in providing immediate assistance in connecting individuals to essential community services and resources, particularly focusing on those at heightened risk of re-hospitalization or suicidal behavior who do not qualify for other services during the assessment process.
Key Responsibilities
- Deliver prompt response and care to individuals transitioning from the hospital, with an emphasis on co-located presence onsite for immediate access to referred individuals.
- Implement short-term, community-based interventions for a duration of up to 90 days following the initial contact.
- Travel as necessary to engage individuals within the community.
- Maintain regular communication with hospital staff to ensure seamless continuity of care, discuss referral workflows, and address any potential issues.
- Conduct follow-up calls and outreach to referred individuals as deemed appropriate.
- Participate in monthly meetings and weekly clinical supervision with the Director of Access & Urgent Care Services.
- Uphold an agreed-upon level of productivity.
- Provide services that are culturally competent, addressing the unique cultural needs of each individual referred.
- Familiarize oneself with all services offered by Compass Health and other human/social service agencies for effective referrals when Compass capabilities are limited, ensuring documentation of such referrals.
- Maintain client records in accordance with agency and state regulations, ensuring all documentation is articulate and professionally written.
- Collaborate with multidisciplinary treatment teams when appropriate, ensuring effective referrals for existing clients.
- A Master’s degree in social work, psychology, or a related field is required.
- A minimum of three (3) years of experience in behavioral health is required.
- Working knowledge of DSM-V is essential.
- A valid MO driver’s license with agency-specified minimum automobile coverage is required.
- Preferred credentials include PLPC, LPC, LMSW, LCSW, PLP, or LP.
