Key Responsibilities
- Conduct comprehensive psychiatric assessments and evaluate physical health needs.
- Facilitate appropriate referrals to community healthcare providers and administer medication under the supervision of the team psychiatrist.
- Offer a spectrum of treatment, rehabilitation, and support services tailored to individuals within the Assertive Community Treatment framework.
- Engage in collaborative efforts with clients and treatment teams to develop, implement, and adjust individualized treatment plans, ensuring responsive and effective care.
- Participate in daily organizational meetings and contribute to treatment planning discussions.
- Maintain thorough documentation in accordance with agency policy, including progress notes and treatment plans.
- Provide on-call crisis intervention during nights, evenings, and weekends as required.
- Assist clients in navigating medical appointments and understanding their treatment plans.
- Strong assessment capabilities and familiarity with psychiatric medications.
- Excellent communication and interpersonal skills to foster collaboration within treatment teams.
- Proficient organizational skills and meticulous attention to detail.
- An Associate’s degree in nursing is required.
- Licensed as an LPN or RN in Missouri is mandatory.
- Relevant experience in an inpatient or outpatient psychiatric setting is essential.
