We are seeking a dedicated Nurse LPN Outpatient to join our integrated care team, committed to delivering exceptional and compassionate care to our primary health care patients. This role is crucial in addressing the comprehensive physical and behavioral health needs of the community, utilizing a person-centered approach. The position is PRN, requiring flexibility to work Monday through Friday during daytime hours, with travel to our Columbia, Marceline, and Salisbury locations as needed.
Key Responsibilities
- Prepare patients for examinations and treatments, including taking medical histories and vital signs, and accurately documenting in patient charts.
- Set up examination and treatment rooms with the required instruments and supplies.
- Execute venipunctures, urinalysis, EKGs, and other related procedures as directed.
- Administer immunizations and injections, as well as phone in prescriptions while documenting accordingly.
- Provide educational support to patients regarding their care instructions and self-management tools.
- Assist physicians and nurse practitioners in the preparation for minor surgical procedures and physicals.
- Manage scheduling for tests and additional treatments.
- Effectively triage telephone inquiries ensuring appropriate referrals and communication of messages.
- Uphold the cleanliness of unit facilities and equipment, and monitor inventory of medical supplies.
- Collaborate with medical providers regarding significant health information and follow up with discharged patients to ensure continuity of care.
- Strong clinical assessment and patient care capabilities.
- Proficiency in healthcare technology and software applications.
- Excellent communication and interpersonal skills, enabling effective collaboration within a team.
- Comprehensive knowledge of medical protocols and procedures.
- High School diploma or GED required.
- Practical Nursing certificate is mandatory.
- A minimum of two (2) years of experience as a licensed practical nurse in a primary health care setting is preferred.
- Familiarity and proficiency with healthcare technology software is favored.
