Key Responsibilities
- Manage a caseload of up to 250 Healthcare Home clients.
- Collaborate with clients to develop and implement annual care plans.
- Assist in formulating treatment goals for individuals with co-occurring chronic conditions.
- Provide educational consultations regarding medical needs and conditions.
- Liaise with medical providers to relay significant health information.
- Educate clients about their medical conditions, treatment options, and medications.
- Monitor health information technology tools and respond to alerts regarding treatment and hospital transitions.
- Conduct medication reconciliations for clients post-hospital discharge.
- Participate in documentation and reporting for UDS measures.
- Work alongside a multi-disciplinary team to ensure comprehensive care.
- Address the unique cultural needs of our diverse clientele.
- Report directly to the Regional Primary Care Nursing Director for clinical oversight.
- Perform additional duties as assigned.
- Proficient in Microsoft Office Suite and Electronic Medical Records.
- Ability to consult and educate healthcare staff on health conditions.
- Strong organizational and communication abilities.
- Knowledgeable in chronic disease management practices.
- High School diploma or GED is required.
- An Associate or Bachelor's degree in nursing is preferred.
- Must possess a Registered Nurse (RN) license in Missouri.
