We are seeking a dedicated and compassionate Community Support Specialist to join our team at Compass Health Network. This full-time position, scheduled Monday through Friday, is vital to addressing the pressing service needs of our communities. By providing integrated, holistic, community-based support services, you will play a crucial role in guiding clients towards their recovery and resilience goals.
Key Responsibilities
- Deliver individualized and group education services tailored to clients’ strengths and needs.
- Collaborate with clients and their families to develop comprehensive Wellness Plans.
- Support clients in achieving their goals as outlined in Wellness Plans, modifying them as necessary.
- Assist clients in accessing essential public services, such as housing, financial aid, and food assistance.
- Maintain consistent communication with hospitalized clients and participate in discharge planning.
- Educate clients on skills essential for daily living, employment, symptom management, and relationship building.
- Facilitate transportation for clients as clinically indicated, ensuring their access to necessary services.
- Engage proactively with clients who have missed appointments to help them re-engage in their recovery journey.
- Effectively document and complete all required paperwork in a timely manner.
- Foster collaboration with the integrated health team, including psychiatrists, primary care providers, and other specialists, to support comprehensive client wellness.
- Strong interpersonal and communication abilities.
- Expertise in providing holistic, client-centered care.
- Familiarity with motivational interviewing and recovery-oriented practices.
- Knowledge of community resources and support services available to clients.
- Ability to work collaboratively within an integrated health team.
- High School Diploma or GED required; an Associate's, Bachelor's, or Master's degree in a human service-related field is preferred.
- Relevant experience required working with individuals facing psychiatric, substance use, or developmental disabilities.
- If a degree is not held, candidates must possess a specified combination of experience and education based on their highest level of education.
- A valid driver’s license and satisfactory automobile coverage are necessary for this role.
