Key Responsibilities
- Conduct triage assessments for individuals presenting at the CAP, who may be experiencing behavioral health distress.
- Emphasize the mission to avoid unnecessary hospitalization and law enforcement involvement.
- When clinically appropriate, carry out comprehensive assessments for agency intake documentation.
- Evaluate the readiness and appropriateness of referrals for internal and external services.
- Lead a multi-disciplinary team while providing supervision to on-shift staff at the CAP.
- Collaborate effectively with team members, Compass Health, and external partners.
- Maintain accurate client records in compliance with agency and state regulations, possessing a strong understanding of utilization management criteria.
- Deliver culturally competent services tailored to the unique needs of each consumer and their family members.
- Establish connections with community partners to facilitate holistic services for individuals served.
- Strong assessment and triage capabilities.
- Ability to work collaboratively within a team environment.
- Familiarity with utilization management criteria.
- Cultural competency in service provision.
- Effective communication and interpersonal skills.
- Excellent organizational skills and attention to detail.
- Must possess Qualified Mental Health Professional (QMHP), Qualified Addiction Professional (QAP), or be a licensed therapist with a minimum of two years of experience preferred.
- Required knowledge of DSM-5.
- Preferred: Master's degree in counseling, social work, or psychology.
- Required: Bachelor's degree in a human services field.
