Compass Medical Provider, LLC is looking for a Registered Nurse Case Manager to support the Idaho Army National Guard. You will be responsible for identifying the Soldiers needs and coordinating with the proper medical facility to ensure that each Soldier gets the care specified.
***Non Clinicial. Office Setting***
The Nurse Case Manager requires expertise of substantial specialized knowledge and skills derived from the biological, physical, and behavioral sciences, to the care, treatment, counsel, and health teaching of individuals who are experiencing changes in the normal health processes or who require assistance in the maintenance of health and the prevention or management of illness, injury, or disability. Case management knowledge domains consist of assessment, planning, implementation, coordination, monitoring, and evaluations of the options and services required to meet the Army National Guards' Soldiers’ health and human service needs.
Qualifications:
- Active, unrestricted nurse license from any U.S. jurisdiction
- Able to meet all licensure requirements
-Active (unexpired) BLS Certificate by the American Heart Association
- A minimum of 2 years' experience with knowledge of military culture, preferred
-Knowledge of common assessment tools used by medical/dental professional's to complete evaluations
-Excellent customer service skills and experience interacting with difficult clientele
- Ability to work independently without supervision and also part of a team
-Preferred skill to have working knowledge of the regulations and policies that apply to Individual Medical Readiness (IMR) and deployment

Responsibilities
- Conduct comprehensive patient assessments to develop individualized care plans.
- Coordinate with healthcare teams to ensure effective discharge planning and continuity of care.
- Provide education and support to patients and families regarding treatment options and resources.
- Maintain compliance with HIPAA regulations and ensure patient confidentiality.
- Utilize managed care principles to optimize patient services and outcomes.
- Assist patients with disabilities, ensuring their needs are met throughout the care process.
