Intake/Admissions Coordinator
The Hospice Intake/Admissions Coordinator is responsible for managing the intake and admission process for patients electing hospice services. This role involves coordinating with patients, families, and healthcare providers to ensure a smooth and compassionate transition into hospice care. The coordinator will manage non-clinical incoming calls, support referral coordination, and maintain accurate records.
In this role, your responsibilities will include:
- Obtain patient history relevant to patient admission to include insurance verification.
- Coordinate with the Admissions Nurse or Clinical Manager to ensure delivery of hospice services.
- Provide emotional support to patients and their families.
- Ensure services are provided in accordance with federal, state, and agency regulations.
- Facilitate communication between patients, their families, and the healthcare team.
- Prepare admission packets.
- Contact Durable Medical Equipment (DME) companies for delivery of equipment.
- Coordinate admissions in a timely manner according to Compass policy and protocol.
- Refer patients to other programs offered by Compass when appropriate.
- Maintains required HIPAA confidentiality of employees, patient/client, and agency information.
Requirements:
- Proficient in completing tasks efficiently and accurately.
- Excellent oral and written communication skills.
- Professional demeanor, able to maintain composure during challenging situations.
- Accountable and able to take ownership of tasks and responsibilities.
- Experience with Electronic Health Records (EHR) systems.
- An ability to cope with the stress of continual loss.
- Must be able to hear instructions in person and by phone.
Support & Training:
- Assist with the job orientation for new employees.
- Assist with staffing needs when required.
Qualifications:
- Minimum of 2 years of experience in a healthcare setting, preferably in hospice care.
- Knowledge of medical ethics and end-of-life care.
- Ability to collaborate effectively with an interdisciplinary team.
- 2 years of experience in computer software operations including MS Office (Word, Excel) and other software packages.
