What are the responsibilities and job description for the Advantage Case Manager position at Compassion Home Care Advantage Program?
We are hiring for a full-time Advantage Case Management position. Advantage Case Managers provide vital services to patients that are members of the Advantage Waiver Program. Case Managers act as an advocate for their assigned patients and assist patients in finding community resources to help them live safely in their homes. Applicants for this position must either be an LPN or RN with a licensure in good standing with the Board of Nursing or have a Bachelor's degree. To be considered for this position, Applicants must have at least one year of experience with the Advantage Waiver Program.
- Initiates IDT meeting on new member as needed.
- Identifies needs and strengths of member.
- Writes individualized goals, actions, and outcomes for each member.
- Writes individualized service plans that are member driven.
- Orients members to the Advantage Waiver Program.
- Obtains member’s signature on necessary forms as needed.
- Completes all paperwork for new member, submits to supervisor for approval, and then to Advantage Operations for authorization.
- Identifies high risk members.
- Coordinates care with other agencies and implements service.
- Completes follow-up on initiation of services.
- Coordinates service delivery.
- Modifies services as member’s condition changes.
- Makes supervisory visits per standard in policy and procedure manual.
- Completes monthly supervisory visits on all high risk members.
- Documents all actions correctly in case notes.
- Educates member on their needs, strengths, and their responsibilities.
