Responsibilities
- Assess needs and develop initial and on-going comprehensive care plans to provide compassionate managed care for our patients.
- Coordinate with multidisciplinary teams to facilitate effective patient care across various settings, including home environments.
- Monitor patient progress and adjust care plans as necessary to meet changing health needs and provide comfort.
- Consulting with and educating patients/families regarding disease process, dietary needs, self-care techniques, and end-of-life care.
- Maintain accurate and up-to-date patient records in compliance with hospice healthcare regulations.
- Communicating information using correct processes and technology available to the organization.
- Perform on-call responsibilities and provide on-call service to patients/families as assigned.
Skills
- Knowledge of end-of-life care, deep compassion and empathy, expertise in pain management.
- Excellent patient service skills with a focus on compassionate patient care.
- Strong organizational skills with attention to detail for documentation and reporting.
