We are currently seeking to fill the Hospice LVN Admissions Coordinator position.
-Position to be performed at the Fresno Office, 93711
-Compensation (negotiable, based on experience, etc.).
-1 year of experience in Hospice required.
Job Summary:
The Licensed Vocational Nurse - Referral Intake Supervisor is responsible for overseeing and ensuring the smooth patient intake process, ensuring compliance with regulatory requirements, and providing high-quality patient care. This role combines clinical expertise with administrative responsibilities, functioning primarily from the office setting.
Key Responsibilities:
- Manage all aspects of the patient intake process, including team supervision, establishing relationships with customers and referral sources, and overseeing insurance verification and authorization.
- Direct daily operations of patient referral and intake, ensuring compliance with state, federal, and organizational standards.
- Maintain positive working relationships with current and potential referral sources.
- Coordinate care with multiple service providers to ensure seamless transition of patients to hospice. Ensuring timely and accurate processing of referrals.
- Documents patient intake process and updates all relevant parties in their communication application of preference.
- Facilitate quick admission processes by ordering medications and coordinating with admission nurses. Receive verbal orders from physicians, ensuring prompt and accurate documentation and implementation of their directives.
- Provide clinical expertise to evaluate patient needs, discussing hospice services with referral sources, patients and families from a nurse's perspective
- Oversee the recruitment, selection, orientation, and management of the referral/intake team.
- Participate in strategic planning and quality assessment activities.
- Utilize nursing knowledge to assess, plan, and evaluate patient care needs.
Qualifications:
- Current LVN license in CA
- Bachelor’s degree in Nursing preferred.
- 1 year of experience in Hospice required.
- Demonstrated ability to supervise and direct both professional and administrative personnel.
- Strong communication, negotiation, and public relations skills.
