Carolina Caring is seeking a dedicated and compassionate Registered Nurse to support and educate patients, families, and caregivers on serious illness and end-of-life care options. This role involves fostering strong relationships between Carolina Caring and local healthcare providers to ensure patients receive the highest level of coordinated care. The Serious Illness Nurse Navigator will perform triage, care management, and care coordination while providing support through home and facility visits and collaborating with the Guide Program to deliver comprehensive care across Carolina Caring’s 12-county service area.
This full-time position includes an excellent benefits package, paid time off, and mileage reimbursement.
Education:
Graduate of an accredited school of nursing – Bachelor’s degree strongly preferred. Must maintain CPR certification.
Experience:
One to three years’ clinical experience required. Hospice or home health care experience preferred.
