Overview:
We are seeking a dedicated Registered Nurse to join our Home Health and Hospice team. The ideal candidate will provide compassionate care to patients in the comfort of their homes.
Duties:
- Conduct in-home patient assessments and develop individualized care plans
- Administer medications and treatments as prescribed by physicians
- Monitor patient vital signs and condition, and report any changes to the healthcare team
- Educate patients and their families on proper care techniques and disease management
- Coordinate with other healthcare professionals to ensure comprehensive patient care
- Document patient progress and treatment plans accurately
Experience:
- Previous experience in patient care, preferably in home health and hospice setting
- Proficiency in medical terminology and basic life support techniques
- Experience with venipuncture and IV therapy
- Familiarity with electronic health record systems
- Knowledge of home care practices is a plus
