Job Overview:
We are seeking a compassionate and dedicated Hospice Nurse to join our team. The ideal candidate will provide quality end-of-life care to patients and support to their families during challenging times.
Responsibilities:
- Conduct patient assessments and develop individualized care plans
- Administer medications and treatments as prescribed
- Monitor and record patient symptoms and vital signs
- Collaborate with interdisciplinary teams to ensure holistic patient care
- Educate patients and families on disease processes, treatment options, and end-of-life care
- Provide emotional support and comfort to patients and families
- Coordinate with healthcare providers to manage pain and symptoms effectively
- Ensure compliance with all regulatory standards and best practices
Requirements:
- Bachelor's degree in Nursing or related field
- Valid RN license in the state of practice
- Proficiency in specimen collection processing and vital signs monitoring
- Experience in home community care, particularly with geriatric patients
- Certification in Basic Life Support (BLS)
