Job Description:
We are seeking a compassionate and skilled Nurse Practitioner to join our hospice team. In this role, you will perform face-to-face visits with patients to assess their needs and provide palliative care support. You will work closely with the interdisciplinary team to ensure patients receive the best care possible during their end-of-life journey.
Responsibilities:
- Conduct face-to-face patient assessments for hospice eligibility.
- Collaborate with doctors, nurses, and other healthcare professionals to create and update care plans.
- Provide compassionate support to patients and their families.
- Document visits and assessments accurately.
- Ensure compliance with hospice regulations and policies.
- Be available during business hours and after hours for staff related needs.
Qualifications:
- Valid Nurse Practitioner license.
- Experience in hospice or palliative care preferred.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a team.
