What are the responsibilities and job description for the Care Coordinator position at CompDrug?
CompDrug is seeking a Care Coordinator, who is responsible for the coordination and care management of CompDrug patients. This includes supporting patients in all phases of care and facilitating connection with resources, problem solving and assisting in navigation of care.

- This is a primarily on-site/in field role with opportunities to work remotely as duties and workflow allow.

Essential Functions
Completes needs assessment outlining support systems and basic and potential needs. Ensures needs identified in assessment are resolved or appropriate services accessed with either internal or external care providers.
Ensures patients have access to all relevant services at CompDrug.
Works as an integral part of the CompDrug care team, and serves to coordinate care, on behalf of the patient, with other healthcare providers.
Primary member of the medication delivery team. Requires inter and intra departmental coordination and communication, driving a company vehicle and delivering controlled medications following protocol and procedures.
Ensures that the care of the assigned patient population is coordinated internally.
Works to ensure that patients receive the treatment they need and will educate patients to ensure they understand the care required for any co-occurring health conditions.
Works with program contact to complete requirements of funders, accreditation bodies, and CompDrug.
Seeks patients exhibiting behaviors indicating potential lapse in treatment and actively works to re-engage in treatment and provides individualized connection and problem-solving support.
Utilizes strong assessment skills and holistic health knowledge to support patients in their treatment with CompDrug and to ensure that their individual needs are met.
Provides support to the patient in a variety of settings - hospitals, appointments, court system. This includes ensuring all necessary Releases of Information are in place and compliant to allow sharing of patient information.
Regular and timely attendance.
Other duties as assigned.
Requirements
Prior care coordination experience strongly preferred.
Strong motivational interviewing and deep listening skills.
Experience with navigating health care systems.
Prefer experience with behavioral health, substance use, pregnant women, and/or Medicaid eligible patients.
Strong interpersonal and group communication skills.
Excellent computer skills.
Excellent collaborative skills.
Attention to detail and experience with data collection procedures.
Demonstrates patience and adaptability in the face of environment and situation fluctuations. Resolution-focused with the ability to think critically and creatively and apply policies and procedures.
CPR certification within 3 months of hire.
Valid driver's license and ability to meet company driver requirements

Work is performed primarily onsite in an outpatient clinic setting and in the community, such as hospitals, court systems, etc.
Operates company vehicle, and transports patients as needed.
Utilizes computer and mobile phone.
