- Serve as the primary contact for families enrolled in the Early Intervention Program, facilitating communication between families, service providers, and other stakeholders.
- Coordinate the delivery of EI services, ensuring all services align with the child’s Individualized Family Service Plan (IFSP).
- Monitor progress and compliance with IFSP goals, providing updates and support as needed.
- Conduct virtual meetings with families and multidisciplinary teams to discuss and review IFSP outcomes and services.
- Ensure accurate and timely completion of all required documentation, including progress reports, service logs, and communication notes, in compliance with NYC EI Program regulations.
- Educate families about available resources and services within their communities and guide them in accessing additional supports.
- Maintain up-to-date knowledge of NYC Early Intervention policies, procedures, and telehealth practices.
- Bilingual in Spanish (required).
- Must live in NYS.
- Must have a Bachelor’s Degree in a "helping" profession
- Minimum of 1 year of experience as a Service Coordinator in the Early Intervention Program.
- Strong understanding of the NYC Early Intervention system, including compliance and documentation requirements.
- Excellent organizational and communication skills, with the ability to manage multiple cases and priorities effectively.
- Proficiency with telehealth platforms and basic technology for virtual service delivery.
