S ocial Worker Qualification s & Re s pon s ibilitie s :
- Mu s t have L S W or C S W
- Previou s long term care experience i s preferred but not required
- Help to identify re s ident’ s p s ycho s ocial , mental, and emotional need s
- Complete S ocial Hi s tory and P s ycho s ocial a s s e s s ment of re s ident s
- Work with interdi s ciplinary team to promote and protect re s ident’ s right s
- Prevent and addre s s re s ident abu s e a s mandated by law and profe s s ional licen s ure
- Ability to build and mai n t ain rapport with re s ident s and peer s
- Ability to work independently or part of a group
- Computer knowledge
- Other dutie s a s a s s igned by Director of S ocial S ervice s and Admini s trator
