Now Hiring: Director of Social Work
Job Type: Full-Time
Responsibilities of Director of Social Work:
- Planning and implementing a comprehensive social services program that offers counseling and other support services for residents and their families.
- Evaluating and being keenly aware of residents’ medical-related social needs and then formulating the appropriate plan for treatment that fits into their total care plan.
- Coordinating and implementing family meetings for residents and creating a forum for ongoing discussions between residents, their families/responsible parties and the treatment team.
- Maintaining residents’ rights at all times while liaising between all parties involved.
- Coordinating discharge planning and assisting in the creation of an organized discharge plan for all residents.
- Ensuring that residents and their families receive the highest quality of service in a caring and compassionate atmosphere that recognizes the individuals’ needs and rights.
Qualifications of Director of Social Work:
- Must have an active BSW.
- Experience in a nursing facility required.
