Job Type: 20 hours
Responsibilities of Social Worker:
- Assist in planning, discharge planning, and identify/organize community resources for residents.
- Work jointly with interdisciplinary team members (i.e., social services, rehab services, and nursing staff) to promote and protect resident rights, dignity, independence, and psychological well-being.
- Ensure the social services department meets all regulatory compliance standards, in accordance with the National Association of Social Workers (NASW), code of ethics, and all state, federal, and local guidelines.
- Complete comprehensive psychosocial assessments for each resident - considering the psychological, social, and emotional needs of each individual resident.
- Spearhead resident-focused trauma-informed care to address individual needs, foster an improved resident experience, and promote high levels of dignity.
- Educate patients and families on their rights and responsibilities. Where applicable, educate patients and families on advanced directives, care planning needs, problem solving, and community resources.
- Communicate with community centers, healthcare facilities, home health, hospice, and palliative care services for resident care plans and discharge planning.
- Bachelor's degree required, preferably in Social Work.
