Job Summary: The Chronic Care Management Enrollment Specialist is responsible for enrolling eligible patients into our Chronic Care Management (CCM) program. This role involves initiating outbound calls, educating patients about the benefits of CCM, and verifying their eligibility. The ideal candidate will have a strong background in customer service, particularly with elderly or chronically ill patients, and will be goal-oriented with excellent communication skills.
Key Responsibilities:
- Initiate outbound calls to eligible patients to introduce and explain the CCM program.
- Educate patients on the benefits of CCM and verify their eligibility.
- Manage patient interactions and maintain accurate records of enrollment status.
- Address patient concerns with empathy and patience.
- Meet enrollment targets and contribute to the overall success of the CCM program.
- Collaborate with healthcare providers and care teams to ensure seamless patient enrollment.
- Maintain confidentiality and adhere to HIPAA regulations.
