Liscensed Practical Nurse

Position Summary: We require a compassionate and experienced Agency Nurse to join our team. The Agency Nurse will be responsible for delivering high-quality nursing care and support to our residents with intellectual disabilities. This role requires a commitment to enhancing the health and well-being of individuals in a residential setting.

Key Responsibilities:

• Oversight: Participate in creating, completing and reviewing physicals, Lifetime Medical Histories, medical records and medical appointment forms at admission/intake and periodically through service provision
• Medication Management: Periodically review medication administration records and active medications for accuracy and monitor responses.
• Health Assessments: Participate in health assessments via Health Risk Screening Tool and document changes in residents’ conditions.
• Care Planning: Collaborate with the interdisciplinary team to develop and update care plans, clinical plans and treatment options; to potentially include attendance at high priority medical appointments
• Communication: Maintain clear and effective communication with staff, residents, and families.
• Documentation: Ensure accurate and timely documentation of all nursing interventions and recommendations.
• Training and Education: Educate residents and staff on health-related topics and additional routes of medication administration
• Compliance: Adhere to all healthcare regulations and standards of practice.

Job Type: Full-time

Pay: $25.00 - $30.00 per hour

Expected hours: 35 – 40 per week
