Duties

- Develop and implement individualized care plans tailored to each resident's needs.
- Ensure compliance with resident rights and promote a respectful living environment.
- Provide guidance and training to staff on best practices in behavior management and medication administration.
- Conduct regular assessments of resident health and well-being, adjusting care plans as necessary.
- Facilitate communication between residents, families, and healthcare professionals.
- Maintain accurate documentation of resident progress and incidents.
- Drive initiatives to enhance memory care programs within the facility.
- Ensure a safe environment by adhering to health and safety regulations.
Skills
- Experience in personal care or ID/D settings is highly desirable.
- Background in social work or related fields is beneficial.
- Proficient in medication administration protocols.
- Strong understanding of individual rights and 6400 regulations
