Position: Case Manager, Behavioral Health

Description: This is an opportunity for a bachelor’s-degreed social worker or counselor to assist people who are challenged by mental illness and/or substance use disorder within a community of great need.
Key Duties:
- Develop relationships with clients and become familiar with their needs
- Know the treatment plan for each client; monitor and assist clients in following the treatment plan
- Follow up with clients to ensure they are attending their mental health counseling, addictions counseling, psychiatric and other appointments
- Help clients to apply for Medicaid and other benefits to which they may be entitled
- Assist clients with applying for Medicaid and other benefits
- Assist clients with managing their household budgets and connecting to representative payee services
- Assist clients with transportation as needed
- Assess and provide referrals to appropriate resources
- Follow up on referrals made by clinicians
- Document services provided within system of electronic health records on same day service is provided to include information needed for medical billing
Reports to: Department Manager
Required:
· Bachelor's degree in social work, counseling, psychology or closely related field
· Good oral and written communication skills
· General knowledge of computer documentation
· Valid driver’s license
Job Type: Full-time
Pay: Up to $20.00 per hour
Expected hours: 40 per week
