Job Description: Intake / Scheduling LPN
__ Non-Exempt Status
_x_ Intake / Scheduling- Nurse – A Licensed Practical Nurse who receives, evaluates, and coordinates referral activity. Verifies third-party reimbursement coverage. Follows up on pending unresolved coverage issues. Performs scheduling activities that facilitate agency operations. Takes Admin / Office On-Call Rotation . Office experience of at least 1 year required. Proficient with Microsoft Office Products.

Essential Functions/Areas of Accountability:
__ Intake Nurse
1. Receives and accurately processes all requests for services.
2. Collects, reviews, and approves Face to Face Visits.
3. Identifies admission criteria for all new patients and evaluates eligibility for home care benefits through third-party payers.
4. Follows agency procedures to obtain prior authorization from third-party payers.
5. Communicates with patients/families regarding coverage for home care services.
6. Maintains a liaison with attending physicians, discharge planners and other referral sources.
7. Works with patients, families and agency staff to problem solve and identify services available to the patient either through CHN or other sources.
___ Scheduling Services
1. Ensures appropriate assignment of referrals.
2. Initiates interdisciplinary referrals.
3. Assures that admissions are completed within established time frames.
4. Coordinates the scheduling of visits. Works with other departments to coordinate professional and para-professional services.
5. Provides appropriate notification of schedule to staff, patients, and others as appropriate.
6. Responds to emergency calls and arranges schedule accordingly.
7. Provides direction to staff and communicates with supervisors as needed to assure safe and effective coverage for patient needs.
8. Coordinates and manages the agency’s daily skilled nursing workload. Coordinates 24-hour nursing coverage.
9. Schedules nursing visits based on patient needs, patient location and staff ability.
___ Paraprofessional Services
1. Coordinates the scheduling of all aide/homemaker services.
2. Processes all admissions for aide/homemaker services. Assures patients’ service needs are filled within established time frames.
3. Provides accurate and timely communication of information and schedules to patients, field staff and office staff.
4. Schedules services based on patient needs identified in the Plan of Care, patient preferences, patient location and staff ability.
5. Works with the Clinical Managers to coordinate professional and para-professional services.
6. Verifies documentation is received for all paraprofessional services.
7. Performs necessary data entry for billing and payroll purposes.
8. Maintains and submits records/reports as required.
9. Keeps schedules current and patient/employee data updated.
10. Monitors staff availability and communicates staffing needs to supervisor.

3. Documents legibly and according to agency documentation guidelines and standards.

1. Demonstrates knowledge of safety/infection control practices by compliance with policies/procedures, and regulatory requirements.
