Job Description: Service Coordinator Manager
__ Clinical Manager – Support Services – Community Liaison/Marketer - Registered Nurse
Assists the Director of Home Care in the management and long-term planning of the patient care area. Directs and develops staff, works with professional and para-professional staff, other managers and administration. Takes Admin / Office On-Call Rotation. This is a 5 day a week position.

Qualifications:
Graduate of an accredited nursing program with a current license as a Registered Nurse in the state(s) of practice. Three (3) years of experience in home care preferred. Supervisory or administrative experience in health care preferred. Knowledge of the regulatory requirements at the state, federal, and local level. Knowledge of reimbursement sources and documentation requirements within home care. Excellent written and verbal communication skills and strong interpersonal skills. Ability to manage diverse projects with a minimum of direct supervision.
Essential Functions/Areas of Accountability:
__ General Management Responsibilities
1. Assists in the planning, development and implementation of the agency structure, the coordination of services, policies and procedures, the annual operating budget and the annual agency evaluation.
2. Manager will be required to provide coverage as needed to make routine visits, including OASIS assessments.
3. Ensures that quality care services are provided within applicable laws and regulations.
4. Uses own knowledge, experience and resources to make logical decisions. Facilitates problem-solving sessions to resolve issues.
5. Serves on the agency Compliance Committee. Participates in agency compliance reviews and monitoring activities.
6. Ensures agency preparation for certification surveys and financial audits.
7. Provides leadership to staff in identifying agency/patient needs and opportunities for quality improvement.
8. Participates in the hiring, orientation and on-going training of personnel.
9. Monitors productivity and performance requirements for staff and participates in correcting any issues.
10. Conducts performance evaluations in accordance with agency policy.
11. Participates in the development and achievement of agency and departmental goals.
12. Performs all duties in an accurate, efficient, cost-effective and timely manner.
13. Confers with the CEO on a regular basis regarding issues that deviate from normal routine.
__ Nurse Management Responsibilities
1. Assists the Director of Home Care in managing and directing Nursing Services to ensure the delivery of safe, effective and appropriate home care services.
2. Provides on-site supervision to determine staff competency, to assess proper use of equipment/supplies, and to receive reports of patient/family satisfaction with care.
3. Ensures accuracy, completeness, and timeliness of clinical documentation in accordance with agency policies/procedures, regulatory requirements, and industry regulations and standards.
4. Identifies and recommends changes in clinical and/or operational practice based on findings of the Quality Improvement Program. Implements changes as directed.
5. Collaborates with the Director of Home Care to review and update clinical policies, procedures, manuals, guides, and forms to improve the quality, efficiency, and effectiveness of patient care at least annually.
__ Support Services - Community Liaison/Marketer
1. Maintains high visibility and availability with referral sources and community groups. Participates in community activities represents agency on boards/community service groups.
2. Serves as a resource to patients, their families, physicians and hospital/nursing home discharge planners. Assures a smooth transition from facility to home care.
3. Manages Intake / scheduling staff and other agency departments to help coordinate needed services for patients.
4. Maintains a knowledge of community services available to patients.
5. Keeps follow-up contact with patients/families on hold.
6. Develops and implements a marketing plan staying within established time table. Reviews and evaluates the plan on an established basis. Makes recommendations for change. Develops marketing materials.
7. Maintains referral information and statistical data. Analyzes areas where increased marketing necessary.

1. Demonstrates knowledge of safety/infection control practices by compliance with policies/procedures, and regulatory requirements.

3. Participates in all mandatory education.
4. Maintains current driver’s license, reliable transportation, and required automobile insurance.
