What are we looking for in a Care Coordinator?
The Care Coordinator for our Certified Community Behavioral Health Clinic (CCBHC) grant supports the physical and mental health of clients by acting as liaison between Comprehensive Healthcare and other community providers to coordinate services and ensure clients can access the resources they need. The Care Coordinator will work closely with all members of the treatment team, from intake to discharge plan, to advocate for clients’ needs and monitor treatment outcomes. This position requires excellent listening and communication skills and the ability to quickly build rapport with people of all backgrounds. The schedule for this position will be Monday-Friday 9am-6pm. This is a grant funded position expected to continue through September 2026.
Care Coordinator duties may include:
• serving as advocate and liaison between the client and the interdisciplinary treatment team, including other medical & service providers in the community
• providing brief interventions using evidence based techniques such as motivational interviewing, behavioral activation, and problem-solving treatment
• collaborating with the client to develop a treatment plan based on the client’s needs and goals
• educating clients about their conditions and the services available to help build a multidisciplinary team to support all aspects of recovery
• tracking response & adherence to treatment and reporting on national outcome measures
• supporting goal of same-day access and first appointment-to-engagement-of-treatment within four business days
• demonstrating knowledge of CCBHC model of care for integrated behavioral health systems and national outcome measures specific to the grant
Qualifications:
• Bachelor’s degree in nursing, social work, social services, or related field
• Experience working with diverse populations and knowledge of social determinants of health
• Driver’s license and vehicle liability insurance
