We’re seeking an experienced Registered Nurse (RN) to join our growing team. If you're ready to be part of a close-knit agency where you can make a lasting impact, this is your opportunity.
What You’ll Do:
- Provide direct, one-on-one skilled nursing care to patients in their homes.
- Complete comprehensive OASIS assessments and develop individualized care plans.
- Monitor patient progress, communicate changes, and coordinate care with the interdisciplinary team.
- Educate patients and families on disease processes, medications, and self-care strategies.
- Ensure documentation is timely, accurate, and in full compliance with Medicare and agency standards.
- Collaborate closely with physicians, therapists, and home health aides to ensure seamless, high-quality care.
- Participate in case conferences and contribute to continuous quality improvement.
Required Qualifications:
- Active Registered Nurse (RN) license in the state of Nevada.

Minimum of 4 years of total nursing experience.

At least 2 years of home health experience in a Medicare-certified home health agency.
- Proficient with OASIS assessments and Medicare documentation requirements.
- Strong clinical judgment, communication skills, and attention to detail.
- Reliable transportation, valid driver’s license, and current auto insurance.
- Familiarity with EMR systems such as Axxess is a plus.
