Responsibilities:
- Conduct thorough assessments of patients' speech, language, cognition, and swallowing abilities, identifying areas of impairment and establishing realistic therapeutic goals.
- Develop and implement individualized treatment plans based on assessment findings, incorporating evidence-based practices to enhance communication and swallowing function.
- Provide skilled speech therapy services to patients in accordance with physician orders, addressing issues such as articulation, voice, fluency, language comprehension, and swallowing disorders.
- Collaborate with interdisciplinary team members, including nurses, occupational therapists, and other healthcare professionals, to ensure coordinated and holistic patient care.
- Educate patients and their families on speech and language exercises, strategies for communication enhancement, and safe swallowing techniques.
- Instruct and guide patients through prescribed therapeutic exercises and activities to improve speech and swallowing function.
- Document patient progress, treatment interventions, and any changes in the patient's condition accurately and timely in accordance with regulatory standards.
- Participate in case conferences and team meetings to discuss patient care plans and contribute to overall care coordination.
- Stay updated on current speech therapy practices, emerging trends, and home health regulations to provide evidence-based and up-to-date care.
- Adhere to ethical and professional standards, ensuring patient confidentiality and fostering a supportive and caring environment.
Qualifications:
- Valid state license as a Speech Therapist.
- Minimum of 1 years of experience in home health speech therapy and/or in an acute care setting.
- Proficient clinical skills with a focus on patient-centered care.
- Excellent communication and interpersonal skills.
- Ability to work independently and collaboratively within a team.
- Understanding of Medicare home health regulations and documentation requirements.
