Responsibilities:
- Conduct thorough assessments of patients' physical conditions, identifying areas of impairment, functional limitations, and setting realistic goals for rehabilitation.
- Develop and implement individualized treatment plans based on assessment findings, incorporating evidence-based practices to maximize patient outcomes.
- Provide skilled physical therapy services to patients in accordance with physician orders, addressing issues such as mobility, strength, balance, and pain management.
- Collaborate with interdisciplinary team members, including nurses, social workers, and other healthcare professionals, to ensure comprehensive and coordinated care.
- Educate patients and their families on therapeutic exercises, safety measures, and home exercise programs to enhance treatment outcomes.
- Document patient progress, treatment interventions, and any changes in the patient's condition accurately and timely in accordance with regulatory standards.
- Maintain open communication with the clinical supervisor, attending regular meetings and participating in case conferences to discuss patient care plans.
- Stay current with advancements in physical therapy practices and home health regulations, incorporating new knowledge into clinical practice.
- Adhere to ethical and professional standards, ensuring patient confidentiality and promoting a positive and compassionate care environment.
Qualifications:
- Valid NV state license as a Physical Therapist.
- Minimum of 1 years of experience in home health physical therapy.
- Strong clinical skills with a focus on patient-centered care.
- Excellent communication and interpersonal skills.
- Ability to work independently and collaboratively within a team.
- Understanding of Medicare home health regulations and documentation requirements.
- Current CPR certification.
- Reliable transportation for home visits.
