About the Role:
As a Per-Diem Occupational Therapist, you’ll provide personalized therapy in a home setting, helping patients regain independence in daily living activities through customized treatment plans and adaptive strategies.
Key Responsibilities:
Conduct comprehensive patient assessments and establish realistic therapy goals.

- Develop personalized treatment plans focused on ADLs, motor skills, cognitive function, and environmental modifications.
- Provide hands-on occupational therapy services, helping patients increase mobility and functionality.
- Educate patients and caregivers on adaptive techniques, energy conservation, and safety strategies.
- Collaborate with nurses, physical therapists, and other healthcare professionals to provide comprehensive care.
- Maintain accurate and timely documentation following regulatory standards.
- Stay current on home health regulations and evidence-based therapy techniques.
Qualifications:
- Valid State License as an Occupational Therapist
- Minimum 1 Year of Experience in home health or acute care
- Strong clinical skills with a patient-first mindset
- CPR Certification (Required)
- Excellent communication & interpersonal abilities
- Reliable transportation for home visits
- Understanding of Medicare home health regulations & documentation
