What are the responsibilities and job description for the Hybrid Health Assistant-Baltimore position at Comprehensive Primary Care?
CloseKnit is a virtual first advanced primary care practice that offers a wide variety of care services, including preventive and urgent care, behavioral and mental health, care coordination, insurance navigation and more. The CloseKnit Health Assistant serves as a bridge between the providers and the non-clinical CloseKnit staff. The Health Assistant maintains efficient patient flow by planning and preparing for patient visits and managing provider calendars. They are also responsible to support an environment of patient safety and satisfaction by developing a relationship with the patients and explaining clinical information. The Health Assistant also supports high-quality care delivery by performing all post-visit tasks, performing quality outreach and demonstrating problem-solving skills around patient needs. This individual functions as a collaborative member of the healthcare team, displaying professional behavior, addressing clinical needs and communicating effectively with other team members and patients.
ESSENTIAL FUNCTIONS:
Clinical Support
· Prior to visit collects and documents new and updated patient information, clinical data and other relevant information such as vital signs, height/weight, allergies, medication reconciliation list, chief complaint, pain score, all past medical, family and social history and current symptoms.
· Requests/obtains additional data from outside sources: EKG/lab/ diagnostic test results, specialist visits and other historical data.
· Schedules patients with urgent needs or abnormal results and arranges referrals.
· Obtain prior authorizations on specific medications, diagnostic testing/imaging, referrals as ordered by provider.
· Performs time-sensitive delegated tasks, which include but are not limited to, patient requests via telephone, email and chat, electronic health record (EHR) messages and triages/addresses phone calls from other providers.
· Completes communication loops to provide feedback and answers to providers, patients, families, pharmacies, suppliers, etc.
· Orders refills and testing per protocols.
· Provides medical record documentation daily.
· Provides age-specific patient education and distributes approved health information materials according to diagnoses.
· Discusses testing results with patients as delegated by the provider.
· Works to create and maintain excellence in patient/family satisfaction.
· Testing and implementation of new workflows and processes to improve patient care and/ or efficiency.
· Outreach to patients to support follow through with care plan and gaps in care.
· Serve as lead for daily provider/health guide/MA huddles
· Works with health guides to divide tasks
· Manages all referrals
· Chaperones virtual sensitive exams.
· Proactive patient outreach where clinically appropriate such as new medication follow up and wellness checks.
· Performs video check ins with patients where appropriate
· Serve as a resource and support for the health guides.
In-person Patient Care
· Prepares, administers and documents oral and injectable medications and vaccines, per provider orders. Consistently follows all elements of medication/vaccine policies.
· Collects laboratory specimens for on-site and remote-site testing. Performs waived lab testing as ordered and per protocols including standardized orders.
· Orders, stocks, and maintains cleanliness of exam rooms and other assigned areas. Documents orders as per protocol.
· Performs and/or assists with in-office procedures, such as EKGs, wound care, PAP smears, etc.
· Operates clinical equipment according to equipment procedures including safety and quality control checks. Reports malfunctioning equipment.
· Conducts point of care testing, orders and refills as per protocol
QUALIFICATIONS:
Education Level: High School Diploma or GED
Experience: 4 years primary care practice performing both front and back-office tasks
Experience must include working with multiple providers
A least 1 year of video or telephone patient outreach experience
AAMA certified or able to be certified within 6 months of hire
Preferred Qualifications:
· Bilingual in English and Spanish
· Care Coordination experience
· Proactive quality improvement experience, such as gap recognition and PDSA cycles
