Job Summary:
We're seeking a skilled Physician Assistant to join our team as a rehab consultant. You'll work closely with our interdisciplinary care team to provide specialized Physical Medicine and Rehabilitation (PM&R) services.
Responsibilities:
- Collaborate with PCP teams to manage conditions common in SNFs, such as CVA, progressive neurological conditions, and postoperative orthopedic surgeries.
- Work alongside PT, OT, and SLP teams to improve patient function and quality of life.
- Follow therapy patients to track their progress in therapy.
- Assess and collaborate on the management of barriers to therapy, including joint or neuropathic pain, spasticity, neurogenic bowel, neurogenic bladder, and more.
- Become an integral part of the SNF team by attending multidisciplinary meetings and bridging the gap between therapy and internal medicine.
