Department : HousingReports To : Director of Housing ServicesType : Full-Time, Salary, ExemptOrganizational Profile : Pridelines Youth Services, Inc., dba "Pridelines" is a rapidly growing 501(c)(3) not-for-profit organization dedicated to supporting, educating, and empowering South Florida’s lesbian, gay, bisexual, transgender, and queer / questioning (LGBTQ ) youth and community, in safe and diverse spaces, to promote dialogue, wellness, and to foster social change.Position Summary : The Youth & Community Case Manager plays a vital role in supporting actively / formerly homeless and unsheltered LGBTQ youth and community members at Pridelines' Transitional and Rapid Re-Housing facility — The Project SAFE House At Rainbow Road Residence. This position requires a dedicated individual with comprehensive knowledge of local / county, state, and federal social services infrastructure, including both public and private resources for under-privileged / under-resourced and in-need individuals, to effectively assist the youth residents and clients who are actively facing significant life challenges. The Case Manager will handle a large caseload (Max. Staff to Client Ratio of 1 : 15), providing personalized support and guidance, helping residents and clients navigate complex social systems and access essential benefits, services, and resources. The Youth Housing Case Manager will be supported in their client navigation activities by a team of mental health professionals, including the Pridelines Housing Therapist, 2-4 Youth & Community Peer Navigators and Youth Behavioral Health Technicians, plus a live-in Residential Assistant — alongside robust guidance from the Executive Director and Director of Housing Services.Key Responsibilities : Case Management : Assess individuals' needs and develop tailored service plans for youth and community member residents and clients, ensuring comprehensive support for their unique circumstances and challenges.Resource Navigation : Guide clients in accessing available resources, including housing / living expenses assistance, food benefits, healthcare services, education, and employment supports.Transportation : Manage, coordinate, and / or directly participate in the transportational needs of Rainbow Road residents from the facility to Pridelines Liberty City Center each day, Monday-Friday, as well as to and from school, work, appointments, etc.Crisis Intervention : Work closely with youth experiencing crises, providing immediate support and connecting them with appropriate programs, services, and resources – both internally and externally – in order to stabilize their situations.Collaboration : Build and maintain relationships with partnering community organizations, government agencies, and homelessness / public health service providers to facilitate resource access for clients.Documentation & Reporting : Utilize Agency EHR / CRM systems : Athena and HMIS — Maintaining accurate and timely records of client interactions, progress, and service outcomes in compliance with Agency and local, state, and federal policies and reporting requirements.Advocacy : Advocate for clients within social welfare systems, including interactions with the Social Security Administration, Department of Children & Families, Department of Labor, local school systems, medical / clinical systems, and job placement services.Workshops & Programming : Assist in the development, planning, and facilitation of activities and workshops aimed at enhancing a broad spectrum of life skills, school / job readiness, and personal development for the youth residents.Crisis Management : Deploy evidence-based approaches and best practices for working with youth who are in crisis, continuously enhancing knowledge, skills, and expertise in trauma-informed care.Professionalism : Maintain strict HIPAA Compliance, confidentiality and discretion in all matters pertaining to clients.Community Center Operation & Administration : Support the day-to-day functions required of a community health / development center setting as assigned, including direct resident, client, patient, and community member engagement and service / resource navigation.Qualifications : Education : Must have a Bachelor’s Degree in Social Work, Psychology, or a related field; relevant experience may substitute for education – Master’s preferred.Experience : Proven experience in case management, social services, or working with youth, particularly those from marginalized backgrounds.Knowledge : In-depth understanding of social welfare systems, including county, state, and federal public services related to housing, education, healthcare, and employment.Skills : Strong organizational skills, with the ability to manage a large caseload effectively.
